New Client Form

Please fill this form out completely and bring it with you to your first appointment with Thomas Vet Clinic.

Boarding Agreement

If your pet will be boarding with us, please fill this form out completely.

Drop Off Form

Fill this form out if you are bringing your pet in for a drop off appointment.

Surgery Consent Form
(Small Animal and Equine)

If you are bringing your pet in for any procedure where he/she will be sedated, please read and sign the following form. Please note the pre-anesthetic blood work portion at the bottom of the page. This is recommended for all procedures, but is not required. Please make sure to sign on the appropriate line to let us know of your decision.

Equine Surgery Complications Form

If your horse will be undergoing a surgical procedure, read the following form carefully, list the value of the horse and sign. This form must be submitted along with the Surgery Consent Form in order for the doctors at Thomas Vet Clinic to operate on your horse.
